Provider Field Descriptions

My Location: You can narrow your search by entering a zip code or city/ state in the My Location filter. Use the search distance to increase or decrease the number of providers displayed in the results.

Facility Name: This is the name of the location where a provider practices. We check with the location to make sure the information is correct when a provider joins and at least every 3 years after that.

Specialties: Providers will have at least one specialty. They may be Board Certified in that specialty but do not have to be.

Languages spoken (in addition to English): All offices speak English.

Gender: As provided by the Provider.

Accepting New Patients: Some providers may not be able to see new patients in their practice. You may have to already be a patient to get an appointment. Providers tell us when they are – and aren’t -- taking new patients. Changes to this information can happen at any time.

Insurance Plans Accepted: Not all providers or facilities accept all insurance plans. Contact the provider's office directly for other insurance plans accepted. For Dental or Vision services, please contact that insurance plan directly.

Provider Office Location(s): This is where the provider sees patients. We update this information when the provider tells us about moves or changes their phone number or office hours. Not all locations provide all services. Contact the location to confirm your service is covered.

Downloading Search Results

Depending on the number of results you are downloading, it could take 1-2 minutes for a PDF of your search results to appear. Each web browser will display the wait differently.

Select at least one filter and the download icon will appear on the right side of your search results.
